          <inserted>1.&amp;#x9;The Coroner is yet to hand down his report into the death of Zhane Chilcott. The timing of the report's release is a matter for the Coroner. The Department for Child Protection cannot intervene in the Coroner's inquest.</inserted>

          <inserted>2.&amp;#x9;The Department for Child Protection (DCP) has taken action in response to Zhane's death from the time it occurred in 2016. While the report of the Coroner is likely to include recommendations for additional action, the department does not wait for a Coroner's report before making improvements to systems, policies or practice when a child in care dies or is seriously injured.</inserted>

          <inserted>DCP has an adverse events review process that identifies important learnings from adverse events relating to a child or young person in care and identifies opportunities for improvement.</inserted>
