          <inserted>Therefore, the inquiry was authorised by David Place in his capacity as chief executive officer, in accordance with government organisational reporting lines.</inserted>

          <inserted>2.&amp;#x9;SAAS has made contact with the families of patients involved in all of the adverse incidents examined by the Hibbert Review in the course of its investigation into systemic clinical management issues.</inserted>
