Has the minister ever received advice about a reportable death pursuant to the Coroners Act, and in particular has the minister at any time received advice that the cluster of nine deaths, which are the subject of the Hibbert report, are or may be reportable deaths pursuant to the Coroners Act, and when was the Coroner first informed of these deaths?</text>
