WADE (Minister for Health and Wellbeing) (15:30):</by>  With all due respect, I think the member is conflating the individual case investigations and the systemic review. Families have been engaged in relation to the individual case investigations. The systemic review isn't a revisiting of that investigation, it's an attempt to look at the systemic issues that are highlighted by an overview of the cases.</text>
        <text id="201902269a28d3d0cb804897a0000322">Associate Professor Hibbert highlighted a whole range of issues, things like clinical supervision, assessment of risk, the maintenance of what I would call clinical observation tools: a very broad ranging set of observations, which I think will be very useful for the health service moving forward to support the maintenance and enhancement of quality and safety.</text>
