Has the Department for Correctional Services' internal review into the circumstances surrounding the failure to readmit Mr Robert Gordon Rigney into custody when he presented himself at Yatala Prison been completed and, if so, has anyone been subject to disciplinary action?</text>

I am happy to seek further advice as to where that particular process is at, but I am happy to convey to the house that I have been briefed on the matter and I am happy to report that a number of measures have been put in place so as to prevent such an instance occurring again. I have stated on the record on more than one occasion, both in this forum and also publicly through the media, that the circumstances that led to Mr Rigney's incident were unacceptable and not good enough, and I have been advised by the department that a number of measures have been put in place that rectify that situation and I am satisfied that that won't happen again.</text>
