On 14 June, the minister described this as 'an error'. On 5 September, the opposition was advised by the Ombudsman that documents were mistakenly determined as outside the scope of an FOI request due to 'human error'. There appears to be a pattern of either deliberate political obstruction or pure incompetence.</text>
