CLOSE (Port Adelaide—Minister for Education and Child Development, Minister for Higher Education and Skills) (14:28):</by>  The standard practice that occurs when a terrible tragedy occurs, such as that murder, if there has been any engagement with the family or any individuals with the department, is that there will be a senior practitioner's internal review of practice. That is then made available to the Coroner.</text>
        <text id="201611303ff5511216124592b0000506">Any further investigation is delayed until after the Coroner has had an opportunity to consider all of the matters and then any subsequent investigations or reviews, such as the child death and serious injury committee, occur after that. So, there has been an internal review that is only for practitioners to determine whether there are any immediate lessons to take home. That is not made public by common practice because it is provided to the Coroner and then is able to be made public through that process.</text>
