          <inserted>The cause of mains failures which occurred in Adelaide's north-eastern suburbs on 7 March 2016 has been investigated by SA Water. The investigation found that an operational issue at the Hope Valley water treatment plant caused a pressure surge in the water main and was the likely cause of the mains failures. </inserted>
