          <item sublevel="1" bullet="true">Phase 1 commenced in 2009 and includes: City of Marion, City of Onkaparinga, City of Playford, City of Salisbury, City of Mount Gambier and the City of Port Augusta.</item>

          <item sublevel="1" bullet="true">Phase 2 commenced in 2010 and includes: City of Port Adelaide Enfield, City of Charles Sturt, City of Whyalla and the District Council of the Copper Coast.</item>

          <item sublevel="1" bullet="true">Phase 3 commenced in 2011 and includes: City of West Torrens, City of Murray Bridge, Mid Murray Council, a combined three council site of District Council of Mount Remarkable, Northern Areas Council and District Council of Peterborough and a second site in City of Playford.</item>

          <item sublevel="1" bullet="true">Phase 4 commenced in 2012 and includes: Alexandrina Council, Coorong District Council, Campbelltown City Council and second sites for City of Salisbury and the City of Charles Sturt.</item>

        <text continued="true" id="201311159a5ed961d0694b20b0000023">The average cost per OPAL site for the five year program is $1,380,000 of which SA Health's contribution is $1,135,000 and the local council contribution is $245,000.</text>
        <text id="201311159a5ed961d0694b20b0000024">Whilst SA Health paid an initial licence agreement fee of $653,070 over four years, from 2009 to 2012, to establish Epode in South Australia, no fees have been paid since. No fee has been paid to join or maintain membership of the Epode International Network.</text>
